Groupmuse COVID-19 Liability Waiver
You are responsible for your own safety. By attending a Groupmuse event, you agree to release Groupmuse, the host, the emcee, all staff, and the performers of all liability for your safety at the event.
I acknowledge that this Liability Waiver will govern my actions and responsibilities at any Groupmuse event. I promise to follow all of the protocols and guidelines put in place by the organizers.
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and other public health authorities still recommend practicing social distancing. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to all who attend and are present at the event and that I come into contact with.
I am not experiencing, nor have I experienced any symptoms of illness in the past 14 days, such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
I have not traveled internationally or to a highly impacted covid-19 area within the USA within the last 14 days.
I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
I affirm that I, as well as those in my household, have not been diagnosed with COVID-19 within the last 14 days. I affirm that I, as well as those in my household, have not knowingly been exposed to anyone diagnosed with COVID-19 within the last 14 days.
I waive, release and discharge from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event or sickness after attending. I indemnify, hold harmless, and promise not to sue the organizers of any Groupmuse event, including staff, hosts, emcees, and performers.
